Methods for Detecting Early Stroke Symptoms: A Literature Review in 2024

Introduction: Early stroke recognition is time-critical, but screening tools vary in symptom coverage and intended setting. Objective: To compare approaches for detecting early stroke symptoms in community, prehospital, and emergency settings. Methods: A narrative literature review examined 15 primary studies and reviews available through 2024, extracting intended users, components, strengths, and limitations of FAST, BEFAST, ROSIER, and prehospital severity scales. Results: FAST is concise for public education and initial triage. BEFAST adds sudden balance and visual symptoms that may be missed by FAST. ROSIER supports structured emergency assessment, while large-vessel-occlusion scales inform referral priority rather than confirm a diagnosis. Conclusion: Tools should match users and settings. Suspected stroke warrants immediate emergency referral and clinical assessment with appropriate imaging.
